Resource Information Request

As part of our project, we are hoping to collect information about parents serving on councils across the country.  We hope to use this information in three ways.  First, it will allow us to communicate with you directly.  Second, we want to connect interested parent leaders with each other so that they may communicate about shared issues (click here for more information about our "leader-to-leader network).  Finally, we want to better understand who the parents are who are typically involved in council work.  We will be happy to share what we learn with you—respecting the confidentiality of the information, of course.

Please download the profile form (click on the link below), print it, and submit it to us either via fax or postal mail.  We ask that you please fill out the profile as completely as you can and then return it to us as soon as possible so that you can begin receiving information about our project.

Please note that the only information we will share with others is your contact information and that is only if you check the box that gives us permission to do so.  We will release the other information but only in the form of a general report that will not have any identifying information whatsoever.  Please attach additional sheets if necessary.
